Come join a dynamic care team at LifeLong Medical Care. We are looking for a Community Health Worker (CHW) for the Care Neighborhood (CN) program. Care Neighborhood is an innovative and interdisciplinary program aimed at sup-porting and empowering identified high risk and need patients in improving their health outcomes. This CHW position will be based out of our Ashby Health Center in Berkeley. The CHW will also work closely with LifeLong’s Street Medicine teams who are providing medical and BH services to people living in homeless encampments or on the streets in Oakland. The CHW will receive the majority of their referrals from the Street Medicine teams and participate in coordinated outreach with the team to build rapport with patients. The CHW will work to help patients become more successfully connected to the clinic and address the social determinants that impact patients’ health, including supporting people who are newly housed maintain their housing. Activities in-clude outreach and engagement, development of a care plan, linkage of the patient with resources (food, housing, transport, financial), as well as linking the patient to care within the clinic and to external specialty care.   Ideally, the CN CHW will be a trusted member of the community in which they will work. This relationship will enable the CHW to serve as a liaison between health and social services and the clinic. Much of this work will take place in various settings – homeless encampments or on the streets, specialists’ offices, hospitals, and patients’ homes. Following LifeLong COVID-19 safety protocols and exercising good judgment regarding risk mitigation are important for both staff and patient safety.    This position functions in partnership with Community Health Center Network (CHCN) in San Leandro. LifeLong Medical Care is looking for a Core Faculty member to join the - Family Medicine Residency Program at the brand new Teaching Health Center in Richmond, the William Jenkins Health Center. As a leader of a newly accredited program, the Core Faculty member will collaborate with the program team to develop a robust outpatient-focused residency program aimed to prepare a diverse group of residents to care for complex patients in urban community health center settings. Under the direction of the Program Director, the core faculty member will serve as clinical leader and resident mentor; assuring the delivery of high-quality teaching and patient care to the underserved safety-net population. Core Faculty members dedicate 40% time to residency training and will provide full-scope primary care in the faculty/residency clinic  60% of the time. Primary responsibility is to ensure the smooth flow of procedures performed by dental staff. Assists dentist(s) in all phases of dentistry, including taking diagnostic intra-oral x-rays and sterilizing instruments.   This is a full time, benefit eligible position. Being bilingual in Spanish is a plus!    LifeLong Medical Care is a large, multi-site, Federally Qualified Health Center (FQHC) with a rich history of providing innovative healthcare and social services to a wonderfully diverse patient community.
