Under the direction of the Office of the CMO, and in collaboration with the Respite Program Director, provide clinical leadership for the LifeLong Medical Respite Program. Care Neighborhood is an innovative program to support and empower identified high risk-high needs patients to improve their health outcomes. Activities include outreach and engagement, development of the care plan, linkage of the patient with resources (food, housing, transport, financial) as well as linking the patient with care within the clinic. The CHW provides services in a variety of locations including the primary care clinic, other medical offices, hospitals and patients’ homes.   This position functions in partnership with Community Health Center Network (CHCN) in San Leandro. LifeLong Medical Care is looking for COVID Testers as part of a patient-centered care team that, along with a COVID Testing Assistant and COVID Scribe, provides COVID-19 services and testing in an outdoor environment to LifeLong Medical Care patients and community members. Under the general supervision of the Center Manager, the COVID Tester is responsible for administering COVID-19 testing to patients, utilizing Electronic Health Record systems, and participating fully in team quality improvement efforts.   As a Federally Qualified Health Center in Northern California with over 42 years of dedicated service to the community, LifeLong serves over 60,000 patients in socio-economically underserved regions of the East Bay Area.  We provide medical, dental, behavioral health, and school-based services at 16 primary care sites across Oakland, Berkeley and west Contra Costa County.  From opening a residential respite program for homeless persons discharged from hospital, to offering free COVID testing to the community, we are innovative and respond to the social and medical needs of our times.  We offer an inclusive workplace that promotes and values diversity and with which our patients can identify.  To this end we seek candidates who reflect the diversity of the East Bay communities we serve.    Benefits: We offer a competitive salary and excellent benefits: medical, dental, vision, Flexible Spending Accounts, dependent and domestic partner coverage, 403(b) retirement savings plan and loan repayment programs. LifeLong is a qualified site for federal and state loan repayment programs (HPSA score is 19).
