Overview

LifeLong Medical Care has an amazing opportunity for a highly motivated Registered Nurse III to join our multidisciplinary team providing exceptional primary care services at our Medical Respite location in Oakland, CA.  The Registered Nurse (RN) - III is an integral member of LifeLong Medical Care’s multidisciplinary Medical Respite team, working alongside medical providers, case managers, and social workers/therapists to deliver high-quality, trauma-informed care. The RN provides direct nursing services to people experiencing homelessness who are recovering from acute illness or injury in a voluntary, short-term residential setting. Responsibilities include clinical assessment, medication management, wound care, care planning, health education, and coordination of follow-up services. The RN plays a critical role in promoting recovery, reducing hospital readmissions, and supporting clients in transitioning to ongoing primary care, behavioral health, and housing resources during their  respite stay.

This position is represented by SEIU-UHW. Salaries and benefits are set by a collective bargaining agreement (CBA), and an employee in this position must remain a member in good standing of SEIU-UHW, as defined in the CBA.

Benefits:

Compensation: $116,000 - $135,,000 year, depending on years licensed. Responsibilities

 

  • Manages the delivery of the health care services at the Medical Respite program site and assists staff in the provision of care.
  • Coordinates services with the multidisciplinary team and support staff engaged in case management, evaluation, education, diagnosis, and treatment of medical needs, drug addiction, and mental health disorders
  • Participates in weekly staff meetings for clinical management, as part of a multidisciplinary managed care team.
  • Consults on clinically and psycho-socially difficult clients in the areas of, but not limited to, wound care, medication management, mental health, nursing procedures, and case management.
  • Consults with MDs, NPs and primary care RNs for ongoing disease care management included but not limited to HIV/AIDS, diabetes, COPD, Hepatitis C, end stage renal disease, and infectious skin diseases.
  • Coordinates with referral sources, ensures referrals are appropriate, and participates in warm handoff to primary care providers for existing residents
  • Promotes effective communication.
  • Performs related functions as required.
  • Coordinates primary care clinic services with other providers and agencies.
  • Coordinates with Care Transitions team for all hospitalized patients to ensure timely follow up and return to primary care.
  • Uses clinician-approved standing orders to order labs and preventive screenings, and to refill routine medications.
  • Coordinates care closely with other team members to identify patient self-care goals and to assure that medical and psychosocial needs are met.
  • Accurately collect information relative to the clients’ condition, assuring proper triage and disposition based on each client’s level of acuity.
  • Effectively perform clinical tasks including, but not limited to: venipuncture, immunizations, providing client education, both individually and in group settings, assisting medical providers with examinations as needed.
  • Regularly assist clients in negotiating the health care system.
  • Effectively collaborate with other team members to maintain an adequate stock of medications and other supplies in the clinic and to review incoming laboratory results to assure providers are notified of any abnormal labs in a timely manner.
  • Regularly seek to update and maintain a current level of knowledge relative to professional practice, and maintain continuing education requirements necessary for licensure.

 

 

Qualifications

  • Qualifications

    • Current, unencumbered license to practice as a Registered Nurse in California
    • Current CPR certification
    • Knowledge of nursing and medical practices, procedures, regulations, and guidelines pertaining to patient care
    • Knowledge of theory, methods and techniques required for rendering patient care
    • Ability to seek direction/approval from supervisor on essential matters, yet work independently with little onsite supervision, using professional judgment and diplomacy
    • Work in a team-oriented environment with a number of professionals with different work styles and support needs
    • Excellent interpersonal, verbal, and written skills and ability to effectively work with people from diverse backgrounds and be culturally sensitive
    • Experience working with diverse patients including the homeless population, disabled persons, and individuals with co-occurring disorders
    • Commitment to the provision of primary care services for the underserved with demonstrated ability and sensitivity in working with a variety of people from low-income populations, with diverse educational, lifestyle, ethnic and cultural origins.
    • Strong organizational, administrative and problem-solving skills, and ability to be flexible and adaptive to change.
    • Work in a team-oriented environment with a number of professionals with different work styles and support needs.

     

    Job Requirements          

    • Bachelor’s Degree in Nursing
    • Current and valid California Registered Nurse license and BLS certification
    • Proficient in Microsoft Office Suite
    • Electronic health records, EPIC preferred
    • 2 years’ nursing experience
    • Demonstrated ability and sensitivity to provide services to persons who are disabled, homeless, substance users, HIV (AIDS) infected, and/or psychologically impaired

    While performing the duties of this job, the employee regularly works in inside environmental conditions.  The employee may be required to attend evening meetings, travel and work evenings or weekends.  The noise level is usually quiet to moderate. Individual is subject to frequent interruptions.
