This position is represented by UAPD. Salaries and benefits are set by a collective bargaining agreement (CBA), and an employee in this position must remain a member in good standing of UAPD, as defined in the CBA.

As a Federally Qualified Health Center in Northern California with over 45 years of dedicated service to the community, LifeLong serves over 66,000 patients in the East Bay Area's socio-economically underserved regions. We provide medical, dental, behavioral health, and school-based services at over 15 primary care sites across Oakland, Berkeley, and West Contra Costa County. From opening a residential respite program for homeless persons discharged from the hospital to offering free COVID testing to the community, we are innovative and responsive to our times' social and medical needs. Benefits:

Compensation: $197,000-$218,000/year for MD/DO and $123,000-146,000/year for APP and pro-rated depending on years since licensed, sign-on bonus, and excellent benefits: medical, dental, vision, Flexible Spending Accounts, dependent and domestic partner coverage, 403(b) retirement savings plan, and loan repayment programs. LifeLong is a qualified site for federal and state loan repayment programs (HPSA score is 21). Acceptance rates may vary year to year based on funding levels and the number of applicants.

 

Responsibilities

  1. Provides preventive, urgent care and primary care health services
  • Performs clinical assessment of patients, including ordering appropriate diagnostic evaluations, and plans a program of care.
  • Provides verbal and written education and counseling to patients, families, and caregivers
  • Plans follow up care including requests for outside consultations and evaluations.
  • Maintains client records necessary for clinic reporting, billing and quality improvement.
  • Consults with the supervising physician as appropriate
  1. Supports and/or participates in triage functions, both by phone and walk in
  • Recognizes and evaluates situations that call for the immediate attention of the physician
  • Provides interim or definitive care
  1. Functions potentially in a variety of settings including clinic, skilled nursing facilities, supportive housing sites, and in clients' homes
  2. Participates as a member of the site clinical services team
  • Attends clinical team meetings
  • Provides specialty care services, clinical case management and team participation to designated programs (e.g., HIV services, women’s health, house calls, diabetes, Bridge, etc.)
  • Coordinates care with other health team members including case management and behavioral health staff
  • Acts as an advocate for the patient and liaison with outside agencies as appropriate
  • Participates in quality performance activities and improvement initiatives
  1. Participates as an active member of LifeLong Clinicians Association and abides by its Code of Conduct.
  2. Actively participate in internal quality improvement teams and work with members proactively to drive quality improvement initiatives in accordance with the mission and strategic goals of the organization, federal and state laws and regulations, and accreditation standards.
  3. Other duties as assigned.

Qualifications

  • Interest in working with a diverse patient population in a community based organization and sensitivity to their needs and health problems.
  • Ability to communicate effectively and work with individuals from a wide range of ages, socio-economic, ethnic, racial backgrounds, and disabilities. Spanish language ability an asset.
  • Ability to work well independently under minimum supervision.
  • Knowledge of Northern Alameda County resources is desirable.
  • Valid California driver’s license, automobile, and automobile liability insurance.
  • Experience with electronic health records, specifically Epic, strongly preferred.

Education and Experience

 

  • Current California license.
  • Community health clinic background preferred.
  • Experience as part of a team-based care model preferred.
