LifeLong Medical Care has an exciting opportunity for a Community Health Worker - Prenatal (CHW) at our Brookside San Pablo Health Center. The CHW's responsibilities include welcoming our patients, making basic psychosocial and nutritional assessments, offering appropriate information, education and referrals, and providing emotional support to a culturally and socioeconomically diverse population. The CHW will work closely with the medical providers and medical assistants. The ideal candidate is someone who likes to work with a team, is familiar with local community resources, and has experience working with women or in a community health setting.
This is a full time, 40 hrs/wk, benefit eligible position. Must be bilingual English/Spanish.
This position is represented by SEIU-UHW. Salaries and benefits are set by a collective bargaining agreement (CBA), and an employee in this position must remain a member in good standing of SEIU-UHW, as defined in the CBA.

Benefits

Patients will receive care that addresses psychosocial, health education, and nutritional needs. Care will be in compliance with the guidelines of the Comprehensive Perinatal Services Program (CPSP).
