Overview

Taking part in community health is rewarding and meaningful. LifeLong Medical Care is searching for a full-time Associate Social Worker (ASW) or Associate Marriage and Family Therapist (AMFT) to work collaboratively with the Behavioral Health staff and operations leadership team to provide effective mental health services to meet the needs of families. The position will require the ability to provide care primarily at our EBCRP Center. This position provides substance use disorder/co-occurring disorder (SUD/COD) assessments and counseling services, transition planning, comprehensive case management, and recovery support services to sentenced adults at Santa Rita Jail with an SUD and/or co-occurring substance use and mental health disorder, or who have been recently released and are returning to their families and community following a period of incarceration at Santa Rita Jail.

This position is represented by UAPD. Salaries and benefits are set by a collective bargaining agreement (CBA), and an employee in this position must remain a member in good standing of UAPD, as defined in the CBA.

 

Benefits:

We offer a competitive annual salary of $68,000-$72,000 for ASW/AMFT, sign on bonus, depending on years since licensed, and excellent benefits: medical, dental, vision, Flexible Spending Accounts, dependent and domestic partner coverage, 403(b) retirement savings plan, and loan repayment programs, and commuter benefits. LifeLong is qualified for federal and state loan repayment programs (HPSA score is 16). Acceptance rates may vary year to year based on funding levels and the number of applicants.

Responsibilities

Essential Functions – Adult Reentry Program (ARP)


• Provides screening and assessments for SUD/COD to individuals within the Santa Rita Jail and at other locations in the community.
• Provides SUD/COD counseling to individuals and groups.
• Provides on-going support and case management services to clients including transition planning, referral to primary care (including HIV, viral Hepatitis (A, B and C) and other infectious diseases), mental health counseling, benefits counseling, food assistance, housing, and referrals for additional SUD/COD treatment, such as Medication Assisted Treatment and residential programs.
• Develops comprehensive service plans and helps coordinate services for/with client and outside referrals services including accessing public benefits and health insurance.
• Communicates with integrated care team members about the program/clients and problems or concerns, including probation officers.
• Completes timely and accurate documentation within EPIC (LifeLong’s electronic health record system) including treatment/service/care plans, progress notes, attendance logs, and discharge planning.
• Participates in all required case conferences, clinical supervision, team meetings, outside meetings and trainings - in person or virtual.

 

Essential Functions – Transition to Treatment (T2T)


• Provides individual and group mental health/substance use disorder (MH/SUD) counseling, assessments, and case management to individuals residing at Alameda County Santa Rita Jail and to eligible participants in the Our Place program.
• Provides follow up to T2T participants post-discharge by providing a warm hand off and assistance accessing services in the community.
• Provides discharge planning services to participants to connect them to appropriate community services.
• Participates in training and oversees administration of the ALOC (ASAM integrated assessment tool) in coordination with the public defender, jail staff, other SUD Portal staff, case managers and mental health staff, for individuals nearing their release date.
• Enters ALOC and summary data in county system.
• Completes timely and accurate documentation within EPIC (LifeLong’s electronic health record system) including treatment/service/care plans, progress notes, attendance logs, and discharge planning.
• Completes required ACBH (Alameda County Behavioral Health) documentation of services provided in the jail.
• Participates in all required case conferences, clinical supervision, team meetings, outside meetings and trainings - in person or virtual.
• Other duties as assigned and deemed necessary and appropriate by supervisors.

Qualifications

• Must be an AMFT/ASW
• At least two years’ experience providing mental health, SUD counseling, and assessments.
• Must pass clearance for Santa Rita Jail
• Experience working with individuals with involvement in the criminal justice system preferred.
• Lived experience is highly desirable.
• Ability to coordinate, develop, and maintain strong relationships with staff, other providers (including those within Santa Rita jail and Alameda County Probation), and community organizations.
• Effectively work in a diverse work environment serving a multi-cultural community, work collaboratively with attorneys, case managers, and probation and jail staff
• Demonstrates ability to work well with people of diverse backgrounds including individuals with mental illness and substance use related issues, those experiencing homelessness, and individuals with a criminal justice background.
• Demonstrated ability and sensitivity in working with a variety of people from low-income populations, with diverse educational, lifestyle, ethnic and cultural origins.
• Maintain professional code of conduct and confidentiality.
• Maintain a problem solving, solution-oriented attitude in all aspects of work.
• Excellent verbal, written and interpersonal communication skills.
• Strong organizational, administrative, and problem-solving skills, and ability to be flexible and adaptive to change.
• Proficient computer skills including using email, Microsoft Word, and the internet. Experience with an Electronic Health Record is helpful.
• Must have reliable transportation to travel between sites in a timely manner, e.g. to Santa Rita Jail and Alameda County Probation sites.
