Overview

Supporting Community Healthcare is a rewarding role. LifeLong Medical Care is looking for Substance Use Disorder (SUD) Counselors/Case Managers at our East Bay Community Recovery Project in Oakland. The primary role of the SUD Counselor/Case Manager is to provide SUD/COD counseling services, comprehensive case management, and recovery support services to individuals with SUD/COD, who are at risk of/or experiencing homelessness.

 

These are full time, 40 hrs/wk, grant funded, benefit eligible positions. 

 

This position is represented by SEIU-UHW. Salaries and benefits are set by a collective bargaining agreement (CBA), and an employee in this position must remain a member in good standing of SEIU-UHW, as defined in the CBA.

Benefits

COVID-19 Vaccine Policy

In accordance with LifeLong Medical Care's commitment to provide and maintain a workplace that is free of known hazards, we have adopted a Mandatory COVID-19 Vaccine Policy to safeguard the health of our employees and their families; our patients and visitors; and the community at large from infectious diseases, that vaccinations may reduce. This policy will comply with all applicable laws and is based on guidance from the Centers for Disease Control and Order of the California State Public Health Officer. Responsibilities

  • Provide assessments and substance use counseling to individuals and groups
  • Provide screening and assessments for Tobacco use
  • Provide outreach and education to individuals at high risk for substance use and homelessness
  • Provide on-going support and case management services to clients including outreach, home visits, and providing advocacy to clients at appointments such as social security, medical, etc.
  • Serve as client advocate, directly facilitate client access to services, and promote self-advocacy
  • Provide employment related services, including but not limited to, on-going work-based vocational assessment, resume writing assistance, job interview skill-building, strength/talent identification, job/career/educational goal identification
  • Provides on-going support and case management services to clients including referral to primary care (including HIV, Hepatitis and other infectious diseases), mental health counseling, benefits counseling, food assistance, housing, and referrals for additional SUD/COD treatment, such as Medication Assisted Treatment, and residential programs
  • Develop comprehensive service plans and help coordinate services for/with client within the service team and outside referrals services including accessing public benefits and health insurance
  • Ensure that all clients’ service plans address identified needs and are current
  • Continually monitor and evaluate each client’s progression through their service plan, and develop corrective action revisions to the plan as needed
  • Communicate with integrated care team members about the program/clients and problems or concerns
  • Maintain accurate documentation of service objectives and outcomes as well as other services in accordance with program guidelines
  • Actively participate in client and program related meetings and trainings
  • Other duties deemed necessary and appropriate by Project Director
  • Responsible to Project Director; clinical supervision provided by licensed counselor.

Qualifications

  • Must be certified or registered as an SUD Counselor by one of the two certifying organizations in California
  • At least two years’ experience providing SUD counseling and assessments
  • Must successfully complete Rapid HIV testing certification
  • Knowledge of HIV/AIDS prevention and treatment programs and targeted outreach
  • Experience working with individuals experiencing homelessness
  • Knowledgeable about community resources, SSI/SSDI, and other public benefits
  • Engages clients in a welcoming, hopeful and empathic manner
  • Demonstrated ability and sensitivity to working with diverse low-income populations with various educational, lifestyle, ethnic, and cultural origins.
  • Well-organized and detail-oriented with excellent verbal and written skills
  • Ability to work well independently and as a member of a team
  • Maintain a problem solving, solution oriented attitude to all aspects of work
  • Valid CA driver’s license required
