Overview

LifeLong Medical Care is looking for a Community Health Worker (CHW) to work in Oakland in the Street Medicine program. The CHW works within a multidisciplinary team including a social worker, registered nurse, and primary care providers. As a key member of this team, the CHW is responsible for outreach, engagement, care plan development, resource connection and coordination of care with people experiencing homelessness and living in encampments, on the street or in other homeless situations. The CHW works with the team and partner organizations to identify client needs, provide immediate access to services and support clients with long term connections to health, housing, case management and other supportive services.

 

This is a full time, benefit eligible position.

Responsibilities

  • Provide field based outreach, engagement and supportive services to individuals experiencing homelessness in encampments, on the streets and other locations.
  • Engage with adults experiencing homelessness and complex physical and behavioral health conditions to develop trusting and mutually respectful relationships.
  • Collaborate and coordinate services with community based organizations including hospitals, benefits advocacy agencies and the coordinated entry system for housing.
  • Advocate on behalf of patients and support them to advocate for their needs and goals.
  • Work collaboratively with each patient to encourage connection to a primary care health home and a long term relationship with health care providers.
  • Provide information and referral to community services such as mental health and housing resources.
  • Assist patients with accessing transitional and permanent housing, public benefits and social services as appropriate.
  • Transport patients, in an agency vehicle, to LifeLong sites and other appropriate locations.
  • Complete all appropriate data collection, paperwork, and tracking of activities in a timely manner.
  • Make appropriate decisions in emergency or crisis situations with awareness of the need for safety of all patients and staff.

  • Strong organizational, administrative and problem-solving skills, and ability to be flexible and adaptive.
  • Ability to effectively present information to others.
  • Ability to work with individuals and organizations at the local level to build support.
  • Ability to seek direction/approval from others on essential matters, yet work independently with little onsite supervision, using professional judgment and diplomacy.
  • Work in a team-oriented, multi-disciplinary environment
  • Excellent interpersonal, verbal, and written skills and ability to effectively work with people from diverse backgrounds.
  • Ability to conduct oneself in external settings to reflect positively on LifeLong Medical Care.
  • Make appropriate use of knowledge/ expertise/ connections of other staff.
  • Be creative and mature with a “can do”, proactive attitude.

Job Requirements

  • High school diploma or GED.
  • 2 years conducting outreach and engagement activities.
  • 2 years providing case management/social services/outreach including harm reduction and motivational interviewing techniques.
  • Proficient in Microsoft office.

Job Preferences

  • BA in social work, psychology or related field or three years relevant experience preferred.
