  • Overview

    LifeLong Medical Care is looking for a Health Promoters Project Specialist to be based out of one of our sites in Richmond, California. S/he will be responsible for overall management of the Health Promoters Program, a grant funded program by The California Endowment Foundation (TCE). Health Promoters are residents who receive specialized training to provide basic health education and information in the community. The goal is to improve long-term health outcomes for the community and promote self-empowerment by training health educators on medical conditions, health and social issues. Bilingual English/Spanish, is HIGHLY desirable

     

    This is a grant funded, full time, exempt, benefit eligible position.

    Responsibilities

    • Responsible for managing overall Health Promoters Program including recruitment, orientation, trainings and year-long service with the organization;
    • Provides oversight and management support to grassroots community health intervention programs as needed;
    • Schedules, implements and attends outreach events, community-based, public health and health promotion, insurance enrollment and other engagement initiatives to support organizational priorities specifically in West Contra Costa County;
    • Manages health promoters schedules with community outreach staff as needed and coordinate work activities of Patient Services Advocate assigned to Health Promoters Grant to carry out necessary community and organizational work;
    • Identifies, initiates, and deepens relationships with patients, community members/organizations, and various internal and external stakeholders;
    • Monitors and manages the Health Promoter Grant primarily and other smaller grants as needed including providing data and narrative to the Planning Department for reporting purposes;
    • Utilizes communication tools to solicit and share feedback from the health promoters about LifeLong Medical Care’s programs and services;
    • Represents LifeLong Medical Care at various community meetings;
    • Attends community meetings and events to better understand community needs and how LifeLong can be a better partner;
    • Creates and gives presentations to community organizations, grantors, community members, board members and other collaborating partners;
    • Maintains directory of relevant community groups and external partners and stakeholders as well as a calendar of key community events;
    • Distributes information both internally and externally to promote an understanding of the health promoter activities to LifeLong staff and community partners.

    Qualifications

    The ideal candidate will have:

    • Bachelor’s degree in related field.
    • Bilingual English/Spanish, HIGHLY desirable.
    • Three years’ experience in nonprofit healthcare setting preferred.
    • Ability to work evenings and weekends regularly to coordinate, attend and/or support staff at community outreach events.
    • Experience working in a fast-paced environment
    • Outstanding organizational skills, attention to detail, and ability to work under pressure with a high degree of accuracy and limited direction
    • Excellent verbal and written communication skills; able to communicate complex ideas simply and succinctly
    • Strong computer and technology skills; proficiency in Microsoft Word and Excel, familiarity with Google docs, and interest in and ability to learn new platforms
    • Ability to work collaboratively with community stakeholders
    • Ability to maintain discretion regarding confidential matters
    • Strong belief in LifeLong’s mission, vision, and core values
    • Valid California driver’s license
