Supporting Community Healthcare is a rewarding role. LifeLong Medical Care is looking for a Program Manager, Population Health to work out of our Administrative Office building in West Berkeley. This is a full time, benefit eligible position. The Program Manager is responsible for developing and overseeing LifeLong’s population health management strategy and program, towards improving the quality of care provided to LifeLong patients and members.
The Program Manager will be hired to start in January 2019. As an attachement with your resume, please include a 1-2 page writing sample in regards to the following: Describe your past experience with developing, launching, spreading, and/or evaluating a public health program. Highlight your role and responsibilities, your approach and how you overcame any challenges with the program’s direction and/or personnel.
