Come join LifeLong Medical Care and support quality healthcare in the community. We are looking for a School Based Associate Clinical Social Worker (ASW) in Oakland. This is a grant funded, exempt, full time, benefit eligible role working for 46 weeks a year for two years (grant ending 5/31/2020). The ASW provides outpatient and group behavioral health services in a school-based clinic setting. LifeLong uses multidisciplinary, integrative team-based care to provide the most efficient and effective care to our patients.

    Responsibilities

    Provides out-patient and group behavioral health services to middle school students in Alameda County.

     

    • Provides individual and group counseling. Crisis intervention as needed.
    • Screens and performs psychosocial assessments.
    • Connects clients and families to other school-based services and community partners when needed. Makes referrals to outside social services agencies as needed.
    • Maintains client records necessary for clinic reporting, billing and quality improvement.
    • Formulates a provisional diagnosis.
    • Consults with behavioral health colleagues and/or additional school-based clinicians on client care when appropriate.

     

    Participates as a member of the site clinical services team.

     

    • Attends staff clinical team meetings
    • Collaborates professionally with interdisciplinary team members and partners including dental, medical and behavioral health providers, health educators, medical assistants, health fellows and school district staff members.
    • Acts as an advocate for the client and liaison with outside agencies.
    • Participates in agency and/or grant driven directives and outcomes.

     

    Actively participates in the culture and climate work group of the school campus.

     

    • Collaborate with work group on existing and future efforts.
    • Offer trauma informed consults to faculty and staff at school site.
    • Facilitate professional development trainings with faculty and staff at school site.

    Qualifications

    • Experience with School-Based Health Care/adolescent health,
    • Experience with an EHR systems (NextGen preferred, not required).
    • Bilingual English/Spanish (Preferred, but not required).
    • Cultural competence in working with a diverse patient population in a community based organization, including trauma informed care.
    • Have flexibility in meeting clients’ needs.
    • Strong knowledge base in mental health and psychosocial issues particular to clinic’s client population.
    • Experience with individual and group modalities, crisis intervention and staff education and training.
    • Excellent interpersonal, verbal, and written skills.

    Required Education and Experience    

    • Master’s Degree in Social Welfare.
    • Minimum of 2-3 years of work experience post MSW in a medical/social service setting providing counseling or therapy and crisis intervention services.
    • Well versed in mandated reporting laws, child development, family systems, stages of change, and co-occurring mental health and substance use/abuse conditions.
    • Proficient in Microsoft office suite
