    Come join a dynamic care team at LifeLong Medical Care. We are looking for a Heart 2 Heart Program Specialist at one of our sites in Berkeley. The Heart 2 Heart Program (H2H) Specialist will implement, manage and evaluate the Heart 2 Heart health service and education program in the South Berkeley Community. They will collaborate closely with the City of Berkeley Public Health Division to bring services to community members as well as the organization of the program. Strong attention to detail as well as above-average interpersonal and organizational skills are necessary for the position.

    Responsibilities

    • Implement, manage and evaluate all aspects of Heart 2 Heart Program;
    • Collaborate with City of Berkeley (COB) partners to initiate plans, objectives, and goals to ensure H2H project meets deliverables;
    • Recruit, train and oversee volunteers and interns for events and specific projects;
    • Attend and participate in meetings and outreach activities with COB partners to ensure H2H project is documenting activities, meeting goals, and reporting requirements in timely fashion;
    • Attend and participates in LifeLong Medical Care outreach and Patient Services staff meetings to ensure coverage for any H2H projects and outreach events in the community;
    • Communicate with COB partners on updates, strategic plans, and communications with outside partners;
    • Establishes and maintains trusting relationships with COB partners, community partners, and community residents;
    • Responsible for management and oversight of Heart 2 Heart Program budget
    • Responsible for data collection, data entry, and generation of reports;
    • Responsible for grant reporting;
    • Provides and links community members to resources when appropriate;
    • Responsible for meeting all deliverables assigned;
    • Generates written communication as directed in a timely and competent manner;
    • Represents LifeLong Medical Care in a manner that is consistent with our goals and vision;
    • Performs other duties and projects as needed or assigned by the Community Engagement Manager.

    Qualifications

    • Excellent interpersonal skills, verbal, and written skills as well as the ability to work with diverse communities with different backgrounds in a culturally appropriate manner;
    • Commitment to the provision of primary care services for the underserved with demonstrated ability and sensitivity in working with a variety of people from low-income populations, with diverse educational, lifestyle, ethnic, and cultural origins;
    • Skills and experience with Microsoft Office programs – Outlook, Excel and Word;
    • Clear understanding of the health issues (hypertension/cardio-vascular disease) plaguing Black and Latino families in this community and related disparities;
    • Commitment to the provision of primary care services for the underserved with demonstrated ability and sensitivity in working with a variety of people from low-income populations, with diverse educational, lifestyle, ethnic, and cultural origins;
    • Display strong customer service orientation;
    • Strong organizational, administrative, multi-tasking, prioritization, and problem-solving skills;
    • Work effectively under pressure and complete assignments in a positive professional and timely manner and to be flexible and adaptive to change;
    • Effectively present information to others, including other employees, community partners, and vendors.
    • Work with individuals and organizations at the local level to build support;
    • Ability to seek direction/approval on essential matters, yet work independently with little onsite supervision, using professional judgment and diplomacy;
    • Work in a team-oriented environment with a number of professionals with different work styles and support needs;
    • Conduct oneself in internal and external settings in a way that reflects positively on LifeLong Medical Care as an organization of professional, confident, and sensitive staff;
    • Ability to see how one’s work intersects with that of other departments of LifeLong Medical Care and that of other partner organizations;
    • Successfully organize and prioritize tasks with little to no oversight;
    • Make appropriate use of knowledge/expertise/connections of other staff;
    • Be creative and mature with a “can do,” proactive attitude and an ability to continuously “scan” the environment, identifying and taking advantage of opportunities for improvement.

    Job Requirements          

    • One to two years’ experience working with a non-profit organization and/or experience in community engagement and health outreach;
    • Must be able to work evenings and weekends;
    • Valid California driver’s license and vehicle;

     

    Job Preferences              

    • Bachelor’s degree or vocational degree in related field;
    • Familiarity with south/west Berkeley community, preferably a resident or someone with strong ties to and passion for this community;
    • Community Health or Non-profit experience.
    • Bilingual in Spanish preferred;
    • Experience with diverse employee populations and multi-location organizations a plus;

     

    Supervisory Responsibility        

    • Supervises Community Engagement Specialist and Fellowship Intern
