The ideal candidate must have a commitment to providing high quality primary care for complex patients, dedicated to building a collaborative team model of care, and experience working with behavioral health diagnoses. Under the general supervision of the Associate Medical Director and Center Manager, the Nurse is responsible for providing clinical care, participate in telephonic and in person patient triage, education and counselling, participating in the ongoing education of the care team and collaborating with medical providers to ensure optimal health of our patient populations.
