    LifeLong Medical Care has an exciting opportunity for a nurse to join our multi-disciplinary team providing primary care services in Alameda/Contra Costa County. The ideal candidate must have a commitment to providing high quality primary care for complex patients, dedicated to building a collaborative team model of care, and experience working with behavioral health diagnoses. Under the general supervision of the Associate Medical Director and Center Manager, the Nurse is responsible for providing clinical care, participate in telephonic and in person patient triage, education and counselling, participating in the ongoing education of the care team and collaborating with medical providers to ensure optimal health of our patient populations.

    Responsibilities

     

    Under the general supervision of the Associate Medical Director and Supervisor, the Nurse is responsible for providing clinical advice and related nursing duties, participating in quality assurance, peer review and multidisciplinary team case management.

     

    Essential duties to include but not limited to:

    • Provides appropriate nursing care, including assessment, triage, and referral, while emphasizing patient comfort and well-being.
    • Coordinates medical care for primary care patients, initiates and follow up diagnostic tests, medical reports, referrals, and emergency care.
    • Participates in quality assurance.
    • Participates in multi-disciplinary team case management.
    • Coordinates primary care clinic services with other providers and agencies.
    • Provides well-documented, accurate and sensitive telephone and in-person drop-in advice, and well as health education and self-care instruction.
    • Participates in community outreach and marketing activities by attending Health Fairs to promote the organization’s activities.
    • Gives presentations on clinic services to interested organizations or civic groups.

    • Ability to effectively coordinate care with medical assistants and other support staff, encourage and nurture development and growth, to build a strong and productive team.
    • Commitment to the provision of primary care services for the underserved with demonstrated ability and sensitivity in working with a variety of people from low-income populations, with diverse educational, lifestyle, ethnic and cultural origins.
    • Strong organizational, administrative and problem-solving skills, and ability to be flexible and adaptive to change.
    • Ability to effectively present information to others, including other employees, community partners and vendors.
    • Ability to seek direction/approval on essential matters, yet work independently with little onsite supervision, using professional judgment and diplomacy.
    • Work in a team-oriented environment with a number of professionals with different work styles and support needs.
    • Excellent interpersonal, verbal, and written skills.
    • Conduct oneself in internal and external settings in a way that reflects positively on LifeLong Medical Care as an organization of professional, confident and sensitive staff.

     

    Job Requirements           

    • Bachelor’s Degree in Nursing.
    • Current and valid California Registered Nurse license and BLS.
    • Proficient in Microsoft office suite
    • Electronic health records, NextGen preferred.

     

    Job Preferences

    • 2 years’ nursing experience
    • Minimum one year experience in community health as an RN.
    • Advanced working knowledge of outpatient care, triage and clinic flow.
    • Demonstrated ability and sensitivity to provide services to persons who are disabled, homeless, substance users, HIV (AIDS) infected, and/or psychologically impaired.
