We are looking for an Intensive Case Manager for our TRUST Health Center in downtown Oakland. This is a full time, 40 hours/week benefit eligible position. Under the supervision of the Associate Medical Director and Supervising LCSW, the Intensive Case Manager will work with an inter-disciplinary team to ensure that quality psychosocial services are delivered to clinic patients.

    • Provides both short and long term intensive case management services to 20 patients who are identified as high priority based on complex medical and behavioral health needs. Provides one new patient intake shift per week. Case management visits are conducted in office and community settings.
    • Completes comprehensive needs assessment and record review for each patient, develops and implements a quarterly care plan which addresses medical and psychosocial needs.
    • Develops and maintain working relationships with outside treatment and other social service agencies, ensuring that clients have maximum access to community resources.
    • Works with team members to provide follow-up assessment, recovery planning, and prevention planning after any crisis, including follow-up with outside agencies.
    • As a team member, assists with crisis intervention and triage as needed. Crisis intervention includes assessment, de-escalation, and notifying the public mental health crisis response team if necessary.
    • Maintains documentation in accordance to billing requirements and submits encounter forms on a weekly basis.
    • Participates in weekly clinical meetings; promotes integrated primary/mental health care in coordination with medical providers and other inter-disciplinary team members.
    • Follows all State of California mandated reporting requirements.
    • Performs other duties as assigned by supervisor.


    • Ability to work effectively and collaboratively in fast-paced primary health care setting and prioritize competing job demands.
    • Must be a self-starter, able to balance case management and administrative tasks.
    • Demonstrated ability and sensitivity to working with a diverse staff and with a patient population from various ethnic, socioeconomic, age, education, lifestyle, and experiential backgrounds.
    • Excellent verbal, written and interpersonal communication skills. Bilingual English/Spanish speaking preferred.
    • Computer literate in MS Office applications and willing to learn clinic systems.

    Education and Experience:

    • Must be a MSW. Supervised ASW hours provided.
    • 1-3 years’ case management experience.
    • Knowledge and experience providing case management services to disadvantaged populations.
    • Familiarity to Alameda County systems of primary and behavioral health care, information and referral, and community supports.
