Come join a dedicated, caring team. LifeLong Medical Care is looking for a Ryan White-funded HIV CASE MANAGER to join our staff at Ashby Health Center in Berkeley. This is a salaried position with the opportunity to work 4 to 5 days a week (32 – 40 hours). The ideal candidate is caring and compassionate with experience in, or an understanding and commitment to, providing services to people of all gender identities and sexual practices, and with histories of mental illness and incarceration.

     

    The HIV Case Manager will provide comprehensive individualized social services to people living with HIV/AIDS (PLWHA) in Alameda County. It is the responsibility of the HIV Case Manager to provide a wide spectrum of services as a part of a primary care team; help to improve the health and well-being of our primary care patients and promote team functioning; ; provide interventions which may include crisis intervention, brief behavioral interventions, psychosocial assessments, collaboration with outside service providers, and appropriate internal/external referrals. A successful candidate will work collaboratively with a core team comprised of one physician, 2 medical assistants / health coaches, a behavioral health CHW and an RN, as well as other relevant clinic staff managing over 100 PLWHA.

    Responsibilities

    Position Responsibilities to include but not limited to:

    • Link and retain patients living with HIV to care at Lifelong.
    • Support linkage to health care services for patients living with HIV (with the goal of self-sufficiency)
    • Support patients’ medication adherence and viral suppression by providing education around relevant lab test results (viral load and CD4 count)
    • Locate and assist patients who have had a lapse in medical care.
    • Educate patients living with HIV in self-care skills.
    • Link patients living with HIV to legal, housing and transportation services.
    • Link patients living with HIV to in-house mental health, substance abuse, and pain management treatments.
    • Link patients living with HIV to medical insurance enrollment specialists
    • Enroll patients living with HIV into the AIDS Drug Assistance Program (ADAP) when necessary
    • Assist in performing rapid HIV tests when indicated
    • Document encounters and units of service using an electronic health record system (NextGen) and the AIDS Regional Information and Evaluation System (ARIES)
    • Maintaining relationships with other clinics/organizations within LifeLong Medical Care and across Alameda County
    • Assist with scheduling speakers and food for monthly HIV Educational Group visits.
    • Participate in weekly case conferences with the HIV Access team.
    • Represent the HIV Access Program at regular meetings of the Alameda County Office of Aids and the HIV Access division of the Community Health Consortium Network.

    Qualifications

    • Bachelor’s degree or equivalent combination of education and experience.
    • Experience providing direct services to clients/patients around sexual health, reproductive health, mental health, etc. preferred.
    • A commitment to serve transgender and gender non-conforming individuals, people of color, women, immigrants (regardless of documentation), and MSM.
    • Knowledge of HIV services in Alameda and/or Contra Costa counties preferred
    • Bilingual Spanish a plus
    • Access to reliable transportation 

     Successful Case Managers must demonstrate:  

    • Excellent verbal and written literacy skills.
    • A commitment to LifeLong’s HIV Program by supporting multiple sites in implementing HIV-related protocols
    • Flexibility and organization of work schedules
    • A harm reduction and trauma-informed care approach when discussing sexual practices, substance use, and medication adherence with patients
    • Empathy
    • Skill in identifying and working with individuals with histories of mental illness, both with establishing clear expectations and maintaining those patients in care.
