Overview

Supporting Community Healthcare is a rewarding role. LifeLong Medical Care is looking for a Temporary Senior Wellness Program Specialist to work at one of our sites in West Berkeley. This is a full time position, working approximately 40 hours/week ending in June 2018. The Senior Wellness Program Specialist will implement, manage, grow and evaluate health promotion and injury prevention services for seniors in Alameda County, including the Home Meds medication safety program, community exercise classes (Tai Chi for Better Balance), and other programs as assigned. Strong attention to detail as well as above-average interpersonal and organizational skills are necessary for the position.

Responsibilities

  • Implement, manage, grow and evaluate LifeLong’s Senior Wellness Programs: Home Meds medication safety program, and evidence-based Tai Chi for Seniors;
  • Implement and manage the Home Meds program, including planning & scheduling with community partners such as senior centers and housing sites, identification and outreach to high-risk patients;
  • Conduct in-home screenings and serve as liaison between clients and pharmacists;
  • Recruit, train and supervise volunteers and interns for events and specific projects;
  • Coordinate and teach community Tai Chi classes for seniors, including scheduling, teacher recruitment and support, and outreach to encourage participation and assess satisfaction;
  • Responsible for establishing, collaborating with, and maintaining effective working relationships with health center colleagues, senior center & senior housing staff, community members, and other key partners;
  • Responsible for management and oversight of Senior Wellness Program Budget;
  • Responsible for data collection, data entry, and generation of reports for funding agency and to assess outcomes/program effectiveness;
  • Provide and link community members to resources when appropriate;
  • Responsible for providing all program-specific grant reporting;
  • Generate written communication as directed in a timely and competent manner;
  • Represent LifeLong Medical Care in a professional manner that is consistent with our goals and vision;
  • Responsible for meeting all deliverables assigned;
  • Performs other duties and projects as needed or assigned by the Community Engagement Manager.

Qualifications

  • Appreciation of older adults and demonstrable aptitude for working successfully with this population; strong motivation to support older adults to achieve their wellness goals;
  • Ability to engage with older adults, especially the underserved, and the community at large in a culturally sensitive manner;
  • Excellent interpersonal skills, verbal, and written skills as well as the ability to work with diverse communities with different backgrounds in a culturally appropriate manner;
  • Must be able to work on weekends;
  • Valid California driver’s license and vehicle;
  • Must successfully complete Home Meds training within 1 month of hire;
  • Commitment to the provision of primary care services for the underserved with demonstrated ability and sensitivity in working with a variety of people from low-income populations, with diverse educational, lifestyle, ethnic, and cultural origins;
  • Display strong customer service orientation;
  • Strong organizational, administrative, multi-tasking, prioritization, and problem-solving skills;
  • Work effectively under pressure and complete assignments in a positive professional and timely manner and to be flexible and adaptive to change;
  • Effectively present information to others, including other employees, community partners, and vendors.
  • Work with individuals and organizations at the local level to build support;
  • Ability to seek direction/approval on essential matters, yet work independently with little onsite supervision, using professional judgment and diplomacy;
  • Work in a team-oriented environment with a number of professionals with different work styles and support needs;
  • Conduct oneself in internal and external settings in a way that reflects positively on LifeLong Medical Care as an organization of professional, confident, and sensitive staff;
  • Ability to see how one’s work intersects with that of other departments of LifeLong Medical Care and that of other partner organizations;
  • Successfully organize and prioritize tasks with little to no oversight;
  • Make appropriate use of knowledge/expertise/connections of other staff;
  • Be creative and mature with a “can do,” proactive attitude and an ability to continuously “scan” the environment, identifying and taking advantage of opportunities for improvement.

Job Requirements          

  • Bachelor’s degree in related field or vocational degree in related field;
  • 2+ years of experience coordinating projects or programs with a non-profit organization and/or in community health outreach, preferably serving older adults;
  • Ability to work out of multiple locations across Alameda County.

 

Job Preferences              

  • Bilingual in Spanish;
  • Experience with diverse employee populations and multi-location organizations a plus;
  • Community Health or Non-profit experience.
