    LifeLong Medical Care is looking for a Supportive Housing Project (SHP) Services Manager. The SHP Services Manager provides administrative oversight for supportive housing case management operations including case manager supervision, caseload management, data collection and reporting, quality assurance, and collaboration with program partners and funders.

    1. Supervise program case managers
    2. Lead regular case conference and team meetings in collaboration with clinical supervisor (LCSW).
    3. Coordinate program data collection and provide management reports for program monitoring and reporting to funders.
    4. Develop and maintain working relationships with outside agencies to promote program referrals and collaboration.
    5. Develop and maintain community resources and linkages.
    6. Assure client referral and intake process supports contract and funding goals.
    7. Develop and manage staff training opportunities.
    8. Develop and submit reports to funders and participate in grant management activities.
    9. Perform other duties as assigned by supervisor.


    • Master’s Degree in a related field (Public Health, Social Work)
    • Experience providing community based support services for special needs populations, especially those with mental health and substance use issues
    • Experience in program planning and management
    • Excellent communication and organizational skills
    • Supervision experience
