    Lifelong Medical Care is looking for a Licensed Clinical Social Worker to work in our Supportive Housing Pogram (SHP) in Oakland and the surrounding areas. This is a part-time benefited position, working 20 hours a week, and helping about 160 mainly formerly homeless adults, consisting mostly of men but some women and children as well. The SHP LCSW provides individual counseling and group facilitation for residents of the permanent supportive housing site(s). As a key member of the on-site services team the LCSW provides clinical support to non-licensed case managers, participates in program planning and supports general program activities The ideal candidate will be professional with excellent customer service, knowledge of medical terminology and data entry, management and reporting experience.

    Responsibilities

    • Provide individual clinical counseling services including assessment, development of client directed service plan and treatment goals.
    • Develop and facilitate client support/therapy groups as appropriate.
    • Collaborate with other members of the service team, property management/landlords, and outside agencies to maximize tenant use of appropriate on- and off-site services and to promote housing stability.
    • Provide clinical consultations for case managers and participate in case conferencing.
    • Assist with crisis intervention.
    • Maintain clinical and administrative records in accordance with contractual and billing requirements.
    • Participate in Quality Improvement processes required for contractual and programmatic implementation.
    • Develop and maintain relationships with community organizations and services.
    • Follow legal requirements for reporting mandated by the State of California.Other duties as assigned.

     

    Qualifications

    • Excellent clinical skills and experience with evidenced based practices for serving SMI adult patients with co-occurring substance use disorders and with histories of trauma.
    • Demonstrated ability and sensitivity in working with a diverse staff and with a complex and diverse patient population.
    • Able to work under pressure and prioritize competing job demands.
    • Excellent verbal, written, and interpersonal communication skills.
    • Ability to effectively supervise para-professional and professional psychosocial staff.

     Licenses and Experience

    • Must hold current LSCW license with California Board of Behavioral Sciences.
    • Experience providing clinical counseling services to individuals with multiple challenges including mental illness, addiction, HIV/AIDS, and other health and social issues.
    • Experience working with low-income and homeless individuals preferred.
