LifeLong Medical Care has an exciting new opportunity for a Quality and Population Health Director. Reporting to the CEO, the Director is responsible for planning, administration and monitoring of all quality management, quality related regulatory requirements, and quality-improvement processes. The Director works in close collaboration with the Chief Medical Officer on clinical priorities and programs.



  • Initiates and oversees the development and execution of a comprehensive quality improvement and population health program and strategy.
  • Coordinates quality improvement initiatives and projects in collaboration with clinical leadership.
  • Prepares the organization for quality and performance review, survey and accreditation processes by external monitoring organizations, such as HRSA, FTCA, CMS, NCQA, or health plans.
  • Works with stakeholders to identify information needs and develop reporting; primary partners include the executive team, management teams, finance and accounting, clinical and administrative operations.
  • Oversees regular reporting of clinical quality and patient experience metrics.
  • Hires, trains, and supervises Quality staff and ensures that the staff meets the needs of the organization.
  • Establishes quality policies, standards, organization, and management.
  • Chairs LifeLong’ s Quality Improvement Committee . 
  • Facilitates CQI Committee of the Board of Directors.

  • Ability to seek direction/approval on essential matters, yet work independently with little onsite supervision, using professional judgment and diplomacy.

Job Requirements   


  • Master’s Degree in healthcare administration or services, business, or other relevant field
  • 5+ years of relevant quality management experience, include improving care and service delivery across an organization and measuring performance.
  • Solid knowledge of and experience in Federally Qualified Health Centers and external quality reporting requirements.
  • Flexibility and strong project leadership skills with the ability to drive large, cross-functional programs to meet strategic and tactical goals.
  • Experience in the use of statistical and quality assessment software.
  • Proficient in Microsoft office suite.


LifeLong Medical Care offers primary care services at 16 health centers that provide an array of medical, dental and behavioral health services. To enhance access to underserved communities, Lifelong also provides care at school health centers, supportive housing locations, urgent/immediate care clinics, an adult day health center and via mobile dental care.
