Overview

LifeLong Medical Care is looking for a Medication Program Coordinator for our Richmond location. The Coordinator works with licensed clinicians and clinic support staff to direct patients to appropriate resources for medications.  Assists providers to expedite prescription refills, assists with pharmacy information requests, manages in-clinic dispensing, and is responsible for ordering, receiving, and managing medications for the clinic dispensary.

 

LifeLong offers primary care services at 16 health centers that provide an array of medical, dental and behavioral health services. To enhance access to underserved communities, Lifelong also provides care at school health centers, supportive housing locations, urgent/immediate care clinics, an adult day health center and via mobile dental care.

Responsibilities

  • Works with patients to determine the most appropriate sources for their prescription medications and refers patients to appropriate programs and resources.
  • Receives referrals for prescription refill assistance from a variety of sources— telephone calls, phone messages, web portal refill requests, and faxed requests from pharmacies.
  • Reviews refill requests and uses LifeLong RN refill protocol to determine whether an RN or the primary care provider should refill the prescription, then tasks appropriate clinician in EHR system to complete or deny the refill.
  • Initiates Prior Authorizations as appropriate.
  • Consults with providers, staff, patients, and pharmacists on issues associated with prescribing medications and makes referrals for recommended on-formulary prescription substitutions, and directs pharmacists to providers for therapeutic substitutions.
  • Provides patient referrals and assistance with applications for pharmacy manufacturers’ drug assistance programs.
  • Assists providers with in-house dispensing. Orders medications, injectable, and medical supplies, and maintains clinic dispensaries that house samples, donations, DAP, and 340B drugs.
  • Works with LifeLong pharmacist to maintain compliance with all local, state and federal pharmacy and dispensary regulations.
  • Other duties as assigned by supervisor.

 

Qualifications

  • Commitment to the provision of primary care services for the underserved with demonstrated ability and sensitivity in working with a variety of people from low-income populations, with diverse educational, lifestyle, ethnic and cultural origins.
  • Strong organizational, administrative, multi-tasking, prioritization and problem-solving skills.
  • Ability to work successfully in a LifeLong team-oriented environment with a number of professionals with different work styles and support needs.
  • Excellent interpersonal, verbal, and written skills both within and outside of LifeLong Medical Care.

 

Job Requirements

  • High school diploma or GED.
  • Minimum of six months experience in a fast-paced pharmacy setting.
  • Valid California Pharmacy Technician certification.
  • Bilingual English/Spanish – based on work site, may be preferred.
  • Knowledgeable in pharmacy terminology, protocols and procedures.
  • Knowledge of program eligibility and insurance coverage issues.
  • Proficient in Microsoft office Word, Excel, Outlook.

 

Job Preferences

  • Bachelor’s degree in related field preferred.
  • Electronic Medical Records (EHR) or pharmacy software experience.
  • Bilingual (Spanish) preferred.
