Overview

LifeLong Medical Care is looking for a part-time 10 hours weekly, Community Health Worker - Behavioral Health for our East Oakland location. Community Health Worker will assist elderly patients (over 55 whose access to essential services is limited by various barriers such as poverty, illness, and social isolation. The CHW will support and promote independent living in a safe environment that enhances and improves quality of life.

 

LifeLong offers primary care services at 16 health centers that provide an array of medical, dental and behavioral health services. To enhance access to underserved communities, Lifelong also provides care at school health centers, supportive housing locations, urgent/immediate care clinics, an adult day health center and via mobile dental care.

 

 

Responsibilities

  • Be an integral part of the Social Services team by providing support for isolated elders in the East Oakland community. The CHW will be supervised by Social Service team professional (LCSW and BSW). On a case by case basis, CHW will transition from close collaboration with staff to independent patient support.
  • Identify priority needs through referrals from the Medical and Social Service team. Examples of needs include: food, transportation, durable medical equipment, and benefit. CHW will support the fulfillment of these needs by completion of forms; follow up with patients and outside agencies. This work will be completed either by phone, at office visits and/or at home visits.
  • Purchase items or services using the Community Health Emergency Fund as appropriate. These include durable medical equipment, transportation assistance, utility bills and other as deemed appropriate by the Social Service team.
  • Track the numbers of patients assisted and the outlay from the Emergency Fund so that we can accurately report our funders.
  • Participate in team meetings with the case manager and medical staff to track needs and follow-ups.           
  • Other duties as assigned.

Qualifications

  • Strong organizational and interpersonal skills with ability to prioritize tasks, work under pressure in a positive, supportive manner.
  • Excellent Verbal and Written skills.
  • Demonstrated computer skills. MS Office proficiency, as well as data base programs.
  • Experience working with a diverse background and people with disabilities.

 

Job Requirement

 

  • Administrative experience in health or social service setting.
  • Knowledge of East Bay health and social service resources.
  • Previous work providing services to persons who are disabled, homeless, substance users, and/or psychologically impaired.
  • Proficient in Microsoft office word with ability to manage databases.

 

Job Preferences

  • Bilingual in English/Spanish a plus
  • 2 years’ experience with essential duties above.
  • High School Diploma
  • Demonstrated ability and sensitivity to provide services to persons who are disabled.
