LifeLong Medical Care is looking for a Clinic Coordinator for our Supportive Housing Program (SHP). The SHP Clinic Coordinator position is a full time benefited role. This position will help coordinate medical clinics at supportive housing sites serving formerly homeless adults. Duties include providing clinical support for the medical providers, developing and maintaining systems to assure efficient administrative and clinical operations and participating in patient outreach, education and follow up.

  • Provides clinical assistance to medical providers at supportive housing site clinics
  • Conducts patient intakes, register new patients and maintain medical records
  • Maintains up to date patient demographic and insurance information in agency clinic management system
  • Manages patient flow during clinic hours
  • Assures that clinics have necessary supplies and are set up prior to scheduled clinic time
  • Maintains logs for immunization in accordance with county and insurance requirements
  • Maintains system for billing and tracking patient encounters
  • Coordinates with case managers, providers and referrals specialist to facilitate referrals for labs, diagnostic tests, specialty care and follow up appointments
  • Works with housing site staff to conduct regular outreach efforts to clients
  • Participates in and helps conduct health education seminars at each site
  • Performs other duties as assigned by supervisor or medical director



  • Ability to work effectively and independently in community based settings
  • Excellent organizational, communication, and interpersonal skills
  • Ability to work independently and effectively under pressure
  • Desire and ability to work with people who have a history of homelessness, mental illness and substance use
  • Access to reliable transportation at least 3 days a week to travel between medical sites

Education and Experience:

  • High school diploma or equivalent required
  • BA in social work, psychology or related field or 2 years relevant experience preferred
