LifeLong Medical Care’s Trust Health Care Center (TRUST) is seeking a Physician, to join our dynamic team in providing whole person care to some of Oakland’s most marginalized communities. This clinic is located in a newly renovated, historic building in the heart of Downtown Oakland. Serving adults currently or recently experiencing homelessness, it aims to create an innovative health care center that responds to patient needs using both licensed and non-licensed staff.  TRUST integrates behavioral health and primary medical care with a focus on meeting patients’ social needs.  The health center accepts people as they are and assists patients in achieving their individual goals for health and wellbeing. The clinic is a multi-agency collaboration between LifeLong Medical Care and Alameda County’s HealthCare for the Homeless program.

  • The Physician reports to the Associate Medical Director and provides direct clinical services while ensuring the delivery of high quality medical and behavioral health services in the primary care setting with a focus on harm reduction and trauma-informed care. 
  • Providing medical care that includes diagnosis, treatment, consultation, medication orders and follow-up care.
  • Interpreting results of clinical tests with the goal of delivering comprehensive care that improves patients' quality of life and prevents unnecessary re-admissions to acute settings.
  • Providing medical care including: screening, health maintenance and disease prevention, management of acute and chronic conditions, provision of vaccinations, and making appropriate referrals and follow-up for diagnostic and specialty services.
  • Working closely with the onsite behavioral health and social services team, which includes case managers and a licensed clinical social workers, and psychiatrists to provide full scope of care. 
  • Maintaining accurate medical records through our electronic health records system.


  • Candidates must be Board Certified or Eligible possessing current CA Medical License.
  • Familiarity with electronic prescribing and electronic health records preferred.  
  • Ability to provide clinical leadership, including quality assurance, oversight of medical staff, and the delivery of clinical services in collaboration with an experienced Clinic Manager.
  • Prior clinical management experience desirable.
  • Demonstrated ability and sensitivity to working with a diverse staff and with a patient population from various ethnic, socioeconomic, age, education, lifestyle and experiential backgrounds, particularly with the homeless population and individuals with mental health and/or substance use concerns.
