LifeLong Medical Care’s Trust Health Care Center (TRUST) is seeking an internal medicine or family practice physician to see adults only committed to providing high quality outpatient primary care working with the under-served. The ideal candidate must be dedicated to building a collaborative team model of care, and ideally experienced working with the under-served. Case management and behavioral health services are fully integrated into our care to address a range of medical and psychosocial factors impacting the overall health of the patient. Spanish speaking is highly preferred but not required.  The Health Center utilizes a multidisciplinary team approach with primary care providers, RNs, Social Workers, Psychiatry, Podiatry, etc. Call is minimal and there is no inpatient work.

  • Providing medical care that includes diagnosis, treatment, consultation, medication orders and follow-up care.
  • Interpreting results of clinical tests with the goal of delivering comprehensive care that improves patients' quality of life and prevents unnecessary re-admissions to acute settings.
  • Providing medical care including: screening, health maintenance and disease prevention, management of acute and chronic conditions, provision of vaccinations, and making appropriate referrals and follow-up for diagnostic and specialty services.
  • Working closely with the onsite behavioral health and social services team, which includes case managers and a licensed clinical social workers, and psychiatrists to provide full scope of care. 
  • Maintaining accurate medical records through our electronic health records system.


  • Candidates must be Board Certified or Eligible possessing current CA Medical License.
  • Familiarity with electronic prescribing and electronic health records preferred.  
  • Ability to provide clinical leadership, including quality assurance, oversight of medical staff, and the delivery of clinical services in collaboration with an experienced Clinic Manager.
  • Prior clinical management experience desirable.
  • Demonstrated ability and sensitivity to working with a diverse staff and with a patient population from various ethnic, socioeconomic, age, education, lifestyle and experiential backgrounds, particularly with the homeless population and individuals with mental health and/or substance use concerns.
