Under the general supervision of the Associate Medical Director, the LCSW will work with an inter-disciplinary team of care providers at our Health Centers in Contra Costa county, to ensure that quality psychosocial services are delivered to clinic patients.

A bilingual in spanish and english candidate is required. Full-time & part-time positions available.


  • Provides comprehensive psychosocial assessments, mental health diagnoses, treatment plans and psychotherapy using appropriate modalities for patients with issues such as: depression, anxiety, trauma, grief/loss, addiction, domestic violence, behavioral health problems, and psychosocial stressors. Works with Community Health Worker to provide short-term case management services for patients participating in treatment.
  • As a team member, assists with crisis intervention and triage as needed. Crisis intervention includes assessment, de-escalation, and notifying the public mental health crisis response team if necessary.
  • Works with team members to provide follow-up assessment and prevention planning after any crisis, including follow-up with outside agencies.
  • Maintains clinical and administrative records in accordance with contractual and billing requirements and submits encounter forms on a daily basis.
  • Participates in team meetings including all staff, provider and behavioral health team meetings; promotes integrated primary/mental health care in coordination with medical providers and other inter-disciplinary team members.
  • Participates in quality improvement efforts related to behavioral health and integrated models of care.
  • Consults informally with providers as needed.
  • Follows all State of California mandated reporting requirements.
  • May serve as supervisor for psychology or MSW interns.


  • Ability to work effectively and collaboratively in fast-paced primary health care setting and prioritize competing job demands. Must be a self-starter, able to balance counseling, case management and administrative tasks.
  • Demonstrated ability and sensitivity to working with a diverse staff and with a patient population from various ethnic, socioeconomic, age, education, lifestyle, and experiential backgrounds.
  • Excellent verbal, written and interpersonal communication skills. 
  • Computer literate in MS Office applications and willing to learn clinic systems.
  • Bilingual English/Spanish highly desirable. 

Education and Experience

  • Must hold current LCSW license with California Board of Behavioral Sciences.
  • Knowledge and experience providing mental health services to diverse, low income populations.
  • Familiarity with evidence-based practices for treatment of behavioral health conditions.
  • Experience in clinical case management.
