Under the direction of a supervising physician, the FNP or PA provides comprehensive outpatient medical care in a primary care setting.  LifeLong uses team-based care to provide the most efficient and effective care to our patients, regardless of insurance or ability to pay. Essential Functions

  1. Provides preventive, urgent care and primary care health services to elementary, middle and high school students in Alameda County.   
  • Provides health education and counseling to patients, families, and caregivers.
  • Makes referrals to specialty providers when appropriate.   
  • Maintains client records necessary for clinic reporting, billing and quality improvement.
  • Consults with the supervising physician and lead school-based clinician on the management, assessment and treatment of specific cases as appropriate.
  • Supports and/or participates in triage functions, both by phone and walk in.
  1. Participates as a member of the site clinical services team
  • Attends staff clinical team meetings
  • Attends school district C.O.S.T. meetings when appropriate
  • Collaborates professionally with interdisciplinary team members and partners including dental, medical and behavioral health providers, health educators, medical assistants, and health fellows. 
  • Acts as an advocate for the patient and liaison with outside agencies as appropriate.
  • Participates in quality performance activities and improvement initiatives
  1. Participates as an active member of LifeLong Clinicians Association and abides by its Code of Conduct. High degree of interpersonal competence and thorough knowledge of evidence based care.
  2. Actively participate in internal quality improvement teams and work with members proactively to drive quality improvement initiatives in accordance with the mission and strategic goals of the organization, federal and state laws and regulations, and accreditation standards.



  • Experience with School-Based Health Care, adolescent health, VFC, CAIR2, FPACT, LARCs insertion, and NextGen and eCW EHR preferred, but not required.
  • Bilingual English/Spanish preferred, but not required.
  • Ability to work well independently under minimum supervision.  Have flexibility in meeting patients’ needs. 
  • Cultural competence in working with a diverse patient population in a community based organization, including trauma informed care.
  • Experience with supervision and training of medical support staff.
  • Valid California driver’s license, automobile, and automobile liability insurance.


Required Education and Experience

  • Nurse Practitioner: Current license from the California Board of Registered Nursing, current furnishing license, a certification from the American Academy of Nurse Practitioners or other national certifying bodies, and a certificate of completion of a Nurse Practitioner Program.   
  • Physician Assistant: Current license from the California Physician Assistant Committee, graduation from an approved program of instruction in primary health care as attested by the American Medical Association, passed certification examination administered by the Bureau of Medical Quality Assurance.
  • Minimum of 1-2 years of work experience in a primary care clinic setting.
