    LifeLong Medical Care is looking for a  PT Licensed Clinical Social Worker. This is your opportunity to work with a community based, primary care clinic serving an underserved patient population. LifeLong Medical Care’s West Berkeley is a Federally Qualified Health Center (FQHC) with a rich history of providing compassionate and innovative team-based care. The West Berkeley site offers integrated Primary and Behavioral Health Care in a family practice serving patients through the life-span.

     

    Under the general supervision of the Associate Medical Director and LCSW Supervisor, the LCSW will work with an inter-disciplinary team of care providers at our West Berkeley Health Center, to ensure that quality psychosocial services are delivered to clinic patients.  The LCSW will ultimately assume management tasks within the clinic.

    Responsibilities

    • Provides comprehensive psychosocial assessments, mental health diagnoses, treatment plans and psychotherapy using appropriate modalities for patients of all ages with issues such as: depression, anxiety, trauma, grief/loss, addiction, domestic violence, behavioral health problems, and psychosocial stressors. Works with Community Health Worker to provide short-term case management services for patients participating in treatment.
    • As a team member may assist with crisis intervention and triage as needed. Crisis intervention includes assessment, de-escalation, and notifying the public mental health crisis response team if necessary.
    • Maintains clinical and administrative records within the EHR system in accordance with contractual and billing requirements and submits encounter forms on a daily basis.
    • Participates in team meetings including all staff, provider and behavioral health team meetings; promotes integrated primary/mental health care in coordination with medical providers and other inter-disciplinary team members.
    • Participates in quality improvement efforts related to behavioral health and integrated models of care.
    • Consults regularly with medical providers and psychiatrists in collaborative care model.
    • Follows all State of California mandated reporting requirements.
    • May serve as supervisor for other LCSWs, CHWs plus MSW interns.

    Qualifications

    • Ability to work effectively and collaboratively in fast-paced primary health care setting and prioritize competing job demands. Must be a self-starter, able to balance counseling, case panel management and administrative tasks.
    • Demonstrated ability and sensitivity to working with a diverse staff and with a patient population from various ethnic, socioeconomic, age, education, lifestyle, and experiential backgrounds.
    • Excellent verbal, written and interpersonal communication skills.
    • Computer literate in MS Office applications and willing to learn clinic systems.
    • Bilingual English/Spanish highly desirable.
    • Must hold current LCSW license with California Board of Behavioral Sciences.
    • Knowledge and experience providing mental health services to diverse, low income populations.
    • Familiarity with evidence-based practices for treatment of behavioral health conditions.
    • Track record of increasing responsibility for coordinating and managing projects and programs.
