    Supporting Community Healthcare is a rewarding role, come join a wonderful organization! LifeLong Medical Care is looking for a Registered Nurse to join our healthcare team in a temporary capacity.  We need a full-time nurse at our Howard Daniel Clinic to cover a leave that is expected to last 3-6 months, possibly more.


    LifeLong Medical Care is a large, multi-site, Federally Qualified Health Center (FQHC) with a rich history of providing innovative healthcare and social services to a wonderfully diverse patient community. Our patient-centered health home is a dynamic place to work, practice, and grow. We have 10 primary care health centers and deliver integrated services including psychosocial, referrals, chronic disease management, dental, health education, home visits, and much, much more.     Under the general supervision of the Associate Medical Director and Triage Supervisor, the Triage Nurse is responsible for providing clinical advice and related nursing duties, participating in quality assurance, peer review and multidisciplinary team case management.


    Essential duties to include but not limited to:

    • Provides appropriate nursing care, including assessment, triage, and referral, while emphasizing patient comfort and well-being.
    • Coordinates medical care for primary care patients, initiates and follow up diagnostic tests, medical reports, referrals, and emergency care.
    • Participates in quality assurance.
    • Participates in multi-disciplinary team case management.
    • Coordinates primary care clinic services with other providers and agencies.
    • Provides well-documented, accurate and sensitive telephone and in-person drop-in advice, and well as health education and self-care instruction.
    • Participates in community outreach and marketing activities by attending Health Fairs to promote the organization’s activities.
    • Gives presentations on clinic services to interested organizations or civic groups.

    Job Requirements       

    • Degree in Nursing.
    • Current and valid California Registered Nurse license and BLS.
    • Minimum one year experience in outpatient clinical practice.
    • Proficient in Microsoft office suite.


    Job Preferences          

    • Advanced working knowledge of outpatient care, triage and clinic flow.
    • Electronic health records experience, NextGen
    • Bilingual – Spanish/English
    • Demonstrated ability and sensitivity to provide services to persons who are disabled, homeless substance users, HIV (AIDS) infected, and/or psychologically impaired.
